This page is for the CareSource Paternal Depression Screening Project at OhioGuidestone. This page should be used by OhioGuidestone clinicians who have been trained to screen fathers. Please refer to emails or training materials for more support and details. There are four forms below that cover all of the needed forms. You will press “submit” after each form, and then return to this page in order to move on to the next form to be filled out.
